ACKNOWLEDGMENT OF RISK

I understand that participation in fitness activities, exercise programs, personal training, group classes, and use of gym equipment at Raw Impact Experience involves inherent risks, including but not limited to muscle strains, sprains, falls, heart conditions, dehydration, equipment failure, serious injury, paralysis, or death. I voluntarily and knowingly assume all risks associated with such activities.

MEDICAL CLEARANCE

I represent and warrant that I am physically fit and have no medical condition that would prevent my full participation in fitness activities. I understand that it is my responsibility to consult with a physician prior to beginning any exercise program and to immediately discontinue activity if I experience pain, dizziness, shortness of breath, or discomfort.

RELEASE OF LIABILITY

In consideration of being allowed to enter the premises and participate in activities at Raw Impact Experience, I hereby release, waive, discharge, and covenant not to sue Raw Impact Experience, its owners, officers, employees, trainers, independent contractors, agents, or affiliates from any and all liability, claims, demands, actions, or causes of action whatsoever arising out of or related to any loss, damage, injury, or death that may be sustained by me or any minor accompanying me, whether caused by negligence or otherwise.

RIGHT TO REFUSE OR TERMINATE MEMBERSHIP

Raw Impact Experience reserves the right to refuse service or terminate membership access at any time, without refund, for unsafe behavior, harassment, policy violations, nonpayment, or conduct deemed inappropriate.

HARASSMENT & CONDUCT POLICY

Raw Impact Experience maintains a zero-tolerance policy for harassment, intimidation, threats, abusive language, discrimination, or inappropriate conduct of any kind toward staff, members, or guests. This includes verbal, physical, and electronic harassment. Violations may result in immediate suspension or termination without refund.

FINANCIAL & BILLING AUTHORIZATION

I authorize Raw Impact Experience to charge all applicable membership fees, recurring dues, late fees, freeze fees, and other approved charges to my payment method on file. I understand that it is my responsibility to maintain current and valid payment information and that failure to do so does not relieve me of payment obligations.

FAILED PAYMENTS & CHARGEBACKS

If a payment is declined, returned, or disputed, Raw Impact Experience may suspend or terminate membership access until the balance is resolved. I agree not to initiate chargebacks for valid charges. Unauthorized chargebacks may result in immediate termination and additional collection or administrative fees.

CHILD & GUARDIAN RESPONSIBILITY POLICY

Raw Impact Experience is not a childcare facility. Parents or legal guardians are solely responsible for the supervision, safety, and conduct of any minors brought onto the premises.

NO PETS / SERVICE ANIMAL POLICY

Pets are not permitted inside Raw Impact Experience. Service animals are permitted only if legally recognized and accompanied by proper documentation. Service animals must remain under handler control at all times.

FREE WEEK TRIAL POLICY

New members are entitled to one (1) free week of training consisting of seven (7) consecutive calendar days beginning on the first day of attendance. Free trial days may not be paused, split, extended, or used non-consecutively. Unused days are forfeited.

CANCELLATION POLICY

Membership cancellations require a minimum of two (2) weeks written advance notice. All cancellation requests

must be submitted in writing via email to Rawimpactadm@gmail.com. Verbal or social media requests will not be accepted as official notice.

MEMBERSHIP FREEZE POLICY

Memberships may be frozen for a maximum of thirty (30) consecutive days at a time. A $10 monthly freeze fee

will apply during the freeze period. Freeze requests must be submitted in writing via email to Rawimpactadm@gmail.com and are subject to management approval. Membership privileges and billing

automatically resume at the end of the approved freeze period.

NO PARTIAL REFUNDS

Raw Impact Experience does not provide partial refunds for unused membership time, missed visits, early cancellations, freeze periods, or unused services, regardless of circumstance.

LOST OR STOLEN PROPERTY

Raw Impact Experience is not responsible for lost, stolen, or damaged personal property brought onto the premises.

VIDEO SURVEILLANCE & SECURITY

The facility utilizes video surveillance for safety, security, and investigative purposes.

INDEMNIFICATION I agree to indemnify and hold harmless Raw Impact Experience from any claims, damages, or expenses arising from my actions.

GOVERNING LAW

This agreement shall be governed by and construed in accordance with the laws of the State of Florida.

I acknowledge that I have read this agreement in its entirety, fully understand its terms, and voluntarily agree to be bound by it.
